The Challenge
Citizen was discharged from hospital following a stroke. On one visit I found the citizen to be leaning so heavily to the side she was at risk of falling out of the chair.
The Change
I completed a seating assessment and made recommendations. I applied for funding for the chair however family decided to purchase it.
The Impact
Independence with functional activities such as eating / drinking and improved communication. The citizen could see out of the window to wave to care staff arriving and leaving improving their wellbeing and quality of life.
