The Challenge
Patient admitted to hospital following a fall and family lived abroad, and unable to visit due to Covid 19 restrictions. Family felt that patient would need to move into long term care as felt she could no longer function at home.
The Change
OT used the Ipad to facetime her family abroad to enable them to observe how she did function on the ward and to discuss in detail discharge plans and services that could be put in place to facilitate her discharge home.
The Impact
Family felt fully involved in her care and in discharge planning and had greater confidence in her ability to cope at home after observing her functional abilities. Patient was able to return, as she wished to and therefore met her goal.
