The Challenge
Mental health inpatients living with dementia had physically recovered from Covid 19 and were being stepped down from the inpatient Covid 19 ward to an unfamiliar ward where not all staff knew them and their background.
The Change
Occupational Therapy provided Personalised Care Booklets tailored to individual and based on their Allen's Cognitive Level. They also contained valuable information on personal interests and recommended approaches for engagement in daily activities.
The Impact
Martha was very distressed by the move. Occupational Therapist helped her to relax using meaningful activity. Martha's Personalised Care Booklet was given to ward. Recognised after by Directorate as valuable and patient centered in challenging times.
