The Challenge
Following a lower limb amputation James was low in mood, deconditioned and struggling to transfer. He was not a candidate for limb-fitting. He thought he would be unable to return home to live alone he thought he would have to move to a care home.
The Change
James participated in a period of inpatient rehabilitation to improve his strength and his ability to transfer with a transfer board and supervision of x1 person. Equipment was provided and then a Home Assessment Visit was carried out.
The Impact
James realised that returning home using a wheelchair was an option. The Home Assessment Visit helped James prepare for discharge and it helped lift his mood when when he found hope.
