The Challenge
Within our acute mental health in patient service, the OT team would regularly signpost patients to community services to further their recovery. However it was evident there were many barriers in place for patients to engage with these services.
The Change
We initiated an 'At the point of care' ethos as part of our established therapeutic programme of activity to work together with partner organisations to offer information sessions and 'drop in clinics' to support patients to access these services.
The Impact
Patients have improved access to community services to support recovery. Including: Smoking Cessation, Drug & Alcohol, IPS Employment, Social Prescribing, Physical Health Checks, Befriending, Carers support, Floating support & Recovery College.
