The Challenge
Patient had many failed discharges prior to coming to our unit. She has a complex mix of brain injury & mental health issues. The challenge was to find a placement to support the patient and to support her to move from the unit and start her new life
The Change
Supporting the patient through her depression with accessible meaningful activity and then supporting through anxiety with graded exposure, promoting success. Supporting her to move on to her new placement, despite her resistance and fears.
The Impact
The patient is now successfully being supported at this placement, she is accessing the community, making new social connections and building positive relationships. She is attending groups, which she had previously disengaged from and is happy!!
