The Challenge
Mrs B was due to be discharged from the community mental health team. One of her outstanding needs was to be able to travel by bus to engage in activities within the local area, which would continue to support her recovery.
The Change
Mrs B attended a six-week New Horizons group (preparing for discharge). In the first session she was able to set specific goals with the support of the group members, including a graded way to use the local buses.
The Impact
Mrs B was motivated to get on the bus independently and this allowed her to first visit the local high street, and then to visit her sister further afield. Her increased confidence will continue to support her to achieve her goals.
