The Challenge
We identified that a number of people were contacting the Council's Care Phones service, sometimes more than 1000 times a month each. This raised concerns about the clients' needs but also the capacity of the service to manage such demand.
The Change
We introduced an OT into the service to undertake assessments of client's needs and identify solutions to manage these. They were able to access equipment, housing, health and social care services and support staff within the service also.
The Impact
By undertaking risk assessments, solutions were put in place to prevent falls, deterioration of health and wellbeing and hospital admissions. Small change in terms of providing equipment had a big impact in reducing the number of calls to the service
