How could we help Mr X referred return home from hospital sooner rather than awaiting lengthy discharge plans, which he was ambivalent about?
The Change
Would traditionally assess people such as Mr X as an in-patient within acute physical healthcare setting, but now able to identify ways to take positive risks and assess people within their own home and the benefits this brings
The Impact
An holistic assessment was carried out looking at a wide range of meaningful Activities of Daily Living and how Mr X uses the home environment to support daily function. This led to Mr X needing less support and reduced time in hospital.
