The Challenge
Gentleman with Parkinsons, ASD and dysarthria was admitted to hospital with a chest infection, there were concerns raised as he wouldn't answer the door to his 4x day carers. It was thought that that he may require 24 hour care to support his needs.
The Change
I did a capacity assessment using a variety of communication aids. We learnt he wasn't taking medication as he didn't know what they were for, & he didn't answer the door because he was out trainspotting & travelling on trains on his mobility scooter
The Impact
He was discharged home with a reduced care package and fully understood why he had to take his medication. Two weeks later he came back (via train!) to visit and thank everyone.
