The Challenge
Transfer method at home unsafe using standaid or mobile hoist, person injuring foot, pain exacerbating dementia symptoms; confusion and disengagement. Wife desperate to support return home, had injured back, distraught as thought would need carehome
The Change
Advised against ward move as each transfer exacerbated confusion, agitation and disengagement in services, making discharge planning very difficult. Investigated ways could support home, installed gantry hoist until ceiling hoist could be installed
The Impact
Wife over the moon able to safely continue to care for her husband avoiding a care home which was costly and detrimental for her husband's wellbeing. The person was content at home and his quality of life optimised, patient centred care
